I'm sure you can always see someone's npub on their profile page, but I'm not exactly sure why you'll need to confirm the age of their npub as this does not necessarily matter.

In a situation where you want them to proof ownership of an account, you can always request they sign a message with their private keys to prove ownership of the npub.

There's always going to be fake accounts whether here or on Twitter, but at least with signature signing, you can verify authenticity here, unlike on twitter.
